Social Services/Admission & Discharge Planner
Knowledge in healthcare settings to coordinate with referral centers for admissions, and discharge planning for smooth transitions of care. Ability to communicate professionally with interdisciplinary team members, patients, and families. Leadership experience and RN/LPN preferred.
